We are looking for a full time, Case Manager for our Medical Team located in Pembroke Pines, FL. This is a Full-Time, Mid-day Shift (12pm to 8pm) in office (Telephonic) position.

Please read on, learn about our benefits below, and only apply if you are able to meet these needs. The main responsibilities of the position include but are not limited to:
Plan, Assess, Implement, Monitor and Evaluate the options and services required to meet the healthcare needs of the patient using Utilization Review, Discharge Planning and Case Management protocols.
Facilitate quality of patient care by utilizing cost effective and outcome management criteria.
Communicate between various departments (internal and external) and hospital / insurance (departments) in order to obtain required information from the health care treating team.
Participate in the active expedition of the movement of patient through appropriate levels of care (repatriation back to country of origin / repatriation to another facility or region that has required services that the patient may need).
Identify appropriate providers and facilities through the continuum of care in order to ensure that resources are provided in a timely and cost effective manner to meet the healthcare demands of the patient.
Required Qualifications for the Medical Case Manager include:
Current and valid license to practice as a Registered Nurse in the state of Florida -For the RN role
Two years of hospital experience in related areas (Emergency Room, ICU, Trauma, Transplant, Neurology, Cardiology, Orthopedics and/or Med/Surg.)
Experience in discharge planning, utilization management, case management, performance improvement and or managed care reimbursement
Computer knowledge should include Microsoft Word and Excel
Strong organizational, time management and team work skills with ability to prioritize and perform multiple tasks
Excellent communication, decision making and problem solving skills
Ability to work independently and exercise sound judgment in interactions with physicians, providers, payors, internal staff and insured's' as deemed necessary
Ability to work with multicultural and diverse population is required.
Must be able to work shifts within 7am to 8pm with preferred shift of 12:00pm to 8:00pm.
Must be able to work weekends, holidays and overnight when on-call.
