About the Opportunity
Spirit Homecare & Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing Home Health team. This role is ideal for nurses who value personalized patient care, meaningful relationships, and making a direct impact in patients’ homes. As part of our interdisciplinary team, you will collaborate closely with Registered Nurses, Therapists, and Physicians to ensure exceptional, patient-centered care.

Compensation & Schedule
Per-Visit Rate: $28 per completed visit (average visit time: 35+ minutes)
Flexible scheduling: Choose between full caseload or PRN visits

Team: Work alongside a collaborative team of RNs, PTs, OTs, and other healthcare professionals

Primary Responsibilities
Deliver direct patient care according to physician orders and care plans
Administer medications, wound care, and prescribed treatments safely and accurately
Document and report patient condition changes to RN Case Manager or physician
Educate patients and caregivers on condition management, medications, and safety practices
Maintain accurate, timely documentation within the EMR system
Assist RNs with specialized nursing procedures
Ensure compliance with agency policies, state, and federal regulations

Qualifications
Active Ohio LPN License (or eligibility for licensure)
Graduate of an accredited Practical Nursing program
1+ year of nursing experience preferred (Home Health experience a plus)
Knowledge of Medicare/CMS guidelines preferred
Proficiency in electronic documentation
Current CPR certification
Valid driver’s license, auto insurance, and reliable transportation

Additional Details
Care Settings: Patient homes, assisted living, and related facilities
Physical Requirements: Ability to walk, bend, lift, and stand for long periods
On-Call: Optional evening, weekend, and holiday rotations
Communication: Maintain regular updates with the Director of Nursing
